Immigration exam request

Dr Sarah Agsten is a designated civil surgeon and offers immigration physical exams and immunization verification for those required to submit an I-693 Form.

Please note, we are not a designated Immigration clinic. We are a private primary care practice offering these exams to support those in the community.

 As such, we require proof of certain requirements prior to being able to schedule your exam and only offer exams on certain days and times. 

Complete Medical Exam Fee: $300.00 Due at time of service

Immunization Verification Only Fee: $100 Due at time of service

*Insurance will not cover these Fee's, however it may cover any vaccination or testing that may be required*
